Quantum Physical Therapy & Chiropractic Care is an outpatient multidisciplinary Rehabilitation Facility dedicated to providing expert, personalized care to all of our patients. With locations in Baldwin, Valley Stream, and Jamaica Estates, you don’t have to travel far to receive the best care possible.
We specialize in helping individuals who have suffered injuries due to automobile accidents, slip and falls, workers compensation traumas, and all other injuries. No matter the cause, our goal is to help everyone who uses our services lessen their pain, increase their function and flexibility, and improve their quality of life.
We believe that a hands-on, one-on-one, personalized approach to care and treatment is our key to success in ensuring every patient we treat sees positive changes and outstanding results. Dr. Jason Birnhak
Chiropractor
After completing his undergraduate education at the University of Maryland College Park and chiropractic schooling at the National University of Health Sciences, Dr. Jason Birnhak began his work as a certified strength and condition specialist in 1996. Shortly after, he began practicing chiropractic care as he worked with many high school, college, and professional athletes to help them heal from their injuries and return safely to their sports.
After practicing in Baldwin for 20 years, Dr. Birnhak became one of the founders of Quantum Physical Therapy & Chiropractic Care, PLLC, in 2012 to help more people receive personalized, expert therapy services.
Johanna Castellano
Physical Therapist
Johanna Castellano’s love for helping others through physical therapy began while studying in college at St. Paul University of Iloilo in the Philippines. After relocating to the United States, she gained valuable experience working with other physical therapists as an assistant and then as a home care physical therapist.
After more than a decade of working and helping others, Castellano became one of the founders of Quantum Physical Therapy & Chiropractic Care, PLLC, in 2012 to continue with her passion and help as many patients receive expert, personalized care as possible.
